We are currently accepting new patients. Appointments can be made by phone at 815-230-2255. Waiting Time: Patients can usually be seen within one to two days of their original phone call.
First Time Patients:
Please bring your insurance card and a photo ID on arriving 15-20 minutes prior to your appointment to allow time for paperwork or click on forms below & complete before your appointment. Please bring a list of your current medications and any pertinent medical/surgical history. Copay is collected at time of visit.
