New Patient Information

We are currently accepting new patients. Appointments can be made by phone at 815-230-2255, Monday – Friday. If you call at a time when we are closed, please do not hesitate to leave a voicemail with your information. We check voicemail multiple times per day and your call will be returned within 24 hours or less.

Waiting Time:

Patients can usually be seen within one to two days of their original phone call
Same-day appointments are welcomed, but are also based on availability
Emergency appointments are available

First Time Patients:

Please bring your insurance information and a photo ID
Plan on arriving 15-20 minutes prior to your appointment to allow time for paperwork
Please bring a list of your current medications and any pertinent medical/surgical history

*IF A PATIENT IS UNDER 18 YEARS OF AGE AND WILL BE ACCOMPANIED BY SOMEBODY OTHER THAN A LEGAL PARENT/GUARDIAN A MEDICAL RELEASE FORM MUST BE FILLED OUT. THAT FILE CAN BE LOCATED HERE.
