We are currently accepting new patients. Appointments can be made by phone at 815.230.2255, Monday – Friday. If you call at a time when we are closed, please do not hesitate to leave a voicemail with your information. We check voicemail multiple times per day and your call will be returned within 24 hours or less. Dr. Bailey can also be paged in emergency situations.

Waiting Time:

• Patients can usually be seen within one to two days of their original phone call
• Walk-in’s and same-day appointments are welcomed, but are also based on availability
• Emergencies appointments available

• Please bring your insurance information and a photo ID
• Plan on arriving 15-20 minutes prior to your appointment to allow time for paperwork
• Please bring a list of your current medications and any pertinent medical/surgical history
